• Leuprolide (Lupron) 1-month depot 7.5 mg IM once every 4 weeks

Example order set #2 (once every 12 weeks)

• Leuprolide (Lupron) 3-month depot 22.5 mg IM once every 12 weeks

Example order set #3 (once every 16 weeks)

• Leuprolide (Lupron) 4-month depot 30 mg IM once every 16 weeks
