HemOnc.org is a reference only to be used by appropriately trained medical professionals. This site is intended to give providers the ability to quickly and refer to the primary literature, notes from themselves and their peers, and to share useful resources.

Users of the site are ultimately responsible for reviewing the primary references for all information. For example, in the case of chemotherapy regimens, users must look up the original references cited to confirm doses and schedules of the constituent medications. Every effort is made to ensure accuracy of the contents within, but it is understood that there may be significant variability in practice among institutions and individuals, and that best practices change over time. Protocols with the same acronym and overall medications are sometimes given with different dosages depending on the study referenced. No liability can be assumed for any typographical or factual errors. Efforts have been made to specifically point out discrepancies in the literature that have been noticed by the authors. Modifications in dose and/or schedule may be needed depending on patients' organ function, performance status, side effects, toxicity, and response. This cannot and should not be interpreted to be an all-inclusive reference, and there is no intention to prioritize or endorse any particular regimen(s).

Example order sets that may contain additional information about supportive medications, suggestions for monitoring, hydration, and pre-treatment screening information for chemotherapy regimens are purely anecdotal, provided only as examples of what some other providers may be using, and are usually not based on references in the primary literature. They are being shared in hopes that they will be helpful for comparison to best local practices and help providers to gain additional insight into variations in practice between clinical sites. Choices for supportive medications, monitoring, hydration, and pre-treatment screening should always be independently determined by providers.
