Morphine milligram equivalent (MME)
- Oral morphine: 1 MME
- Hydrocodone: 1 MME
- Oxycodone: 1.5 MME
- IV morphine: 3 MME
- Oral hydromorphone: 4 MME
- IV hydromorphone: 20 MME

301-500 mg/day Morphine --> 12:1 of Methadone
501-1000 mg/day Morphine --> 15:1 of Methadone
>1001mg/day Morphine --> 20:1 of Methadone
