Note: There are disagreements about the accuracy of these conversions, which were mostly established in healthy persons. If and when guidelines on this topic are available regarding cancer-related pain, we will add them here.
Morphine milligram equivalent (MME)
- Oral morphine: 1 MME
- Hydrocodone: 1 MME
- Oxycodone: 1.5 MME
- IV morphine: 3 MME
- Oral hydromorphone: 4 MME
- IV hydromorphone: 20 MME
Conversion table using common doses
Opioid | IV/SubQ/IM (mg) | PO/Rectal (mg) |
---|---|---|
Morphine | 10 | 30 |
Hydrocodone | - | 30 |
Oxycodone | - | 20 |
Hydromorphone | 1.5 | 7.5 |
Codeine | - | 200 |
Hydrocodone | - | 30 |
Tramadol | - | 100 |
Fentanyl* | 0.1 | - |
*Fentanyl dosing is only for intermittent bolus dosing.
To convert Fentanyl boluses to Fentanyl patch, please see example opioid conversions.

Up to 100 mg/day Morphine --> 4:1 of Methadone
101-300 mg/day Morphine --> 8:1 of Methadone
301-500 mg/day Morphine --> 12:1 of Methadone
501-1000 mg/day Morphine --> 15:1 of Methadone
>1001mg/day Morphine --> 20:1 of Methadone
