  • Methotrexate (MTX) by the following age-based criteria:
    • Less than 2 years old: 1.25 mg PO once per day
    • 2 to 10 years old: 2.5 mg PO once per day
    • Older than 10 years old: 5 mg PO once per day

Continued indefinitely
